What actually causes this?
Fabric moving against skin over hours, made worse by salt, cold and a shirt that is wet and heavy. Seamless technical fabric, a snug fit and lubricant help. A cotton shirt over a long distance is the classic cause.
Does anti-chafe balm work instead?
For some people, for a few hours. Over a very long day it wears off, which is why tape and covers exist. Many runners use balm everywhere else and a physical barrier here.
What if it is already raw?
Cover it, stop the friction, and keep it clean. Skin that is broken, spreading redness, or weeping needs medical attention rather than more tape, and the same goes for anything that has not started healing within a couple of days.
