The short version

Ice is what cools you; the towel just holds water. An evaporative towel works by wetting and evaporation, which does very little once the air is already humid. A bandana packed with ice puts something genuinely cold against the neck and keeps working as it melts. In a hot race the deciding factor is whether aid stations have ice, and in most summer ultras they do. Neither substitutes for slowing down when it is dangerously hot.

Does cooling the neck lower core temperature?

Only slightly. What it reliably does is reduce perceived effort and thermal discomfort, which in practice lets people keep moving. Real core cooling comes from ice in a hat, ice down the shorts, cold water immersion and, more than anything, from slowing down.

What are the warning signs of heat illness?

Stopping sweating, confusion, stumbling, unusual aggression or apathy, nausea and a rapid pulse. Exertional heat stroke is a life threatening emergency and the treatment is rapid whole body cooling, usually cold water immersion, plus emergency medical help. Do not let anyone showing these signs continue.

How do I train for heat?

Gradual heat acclimation over one to two weeks of exposure produces real physiological adaptation. Doing it accidentally on race day does not. If you have a heart condition or take medication affecting heat tolerance, ask a doctor before deliberate heat training.
